Youth in Transition Worker (YITW) help young adults develop and pursue their goals and will support them to identify, access and navigate through the adult services. The YITW will also support the youth to connect to existing supports and resources within their communities. This may include education, housing, employment/training services, life skills training (e.g. budget/household management), medical health, mental health services and legal services.
